Question Prep for Scheduling phone call

Did your child receive early intervention services?

We want to make sure we have a clear picture of your child's development. Input and history from previous or current therapists can be really helpful for our team.

What school/daycare does your child attend?

In an effort to gain a holistic understanding of how your child moves through their world, we will also seek input and observations from your child's instructors. Please let us know what school or daycare they attend. Please also let us know if your child has an IEP. 

Has your child or does your child receive speech therapy, occupational therapy, physical therapy, or

We will spend several hours with your child but when children are engaged in ongoing therapies it is likely that clinician will have really good insight and observation into your child's functioning overtime and in a variety of environments. Their input can be really helpful as well.

Have you been in contact with GIDES or The Arc of Whatcom County?

GIDES and The Arc of Whatcom County are committed to supporting families in Whatcom county with neurodivergent children. They want to offer you support through this process and make sure you don't feel alone. If you would like to be connected with them, please let us know and we would be happy to facilitate that.

Is there anyone else you would like us to gather information from before your appointment?

Each family's "village" can look a little different. If there are important people in your child's life that we should talk to before the evaluation, please let us know!

Release of Information

During the phone call with our scheduler, we will add anyone mentioned in the conversation above to a release of information form and email it to you to get a signature. This will allow us to start gathering important information so we're ready when you and your child come to the clinic.
